Through a range of home and community based treatment and support services, our ACT team helps individuals with severe mental health conditions better integrate themselves into their community.

Team Approach

Our ACT Team consists of: a psychiatric care provider, nurses, vocational specialist, co-occurring disorder specialist, program administrative assistant, certified peer support specialist, mental health practitioners, and ACT team lead.

Continuous, Personalized Care

Various team members with small cases loads ensure the best possible care. Support is provided frequently and daily if necessary. Staff adjust their schedules in order to be flexible with client needs.

The majority of team contacts are within a community setting.

Frequently Asked Questions

  • Adults (18+) who experience severe and persistent symptoms of mental illness and have a primary diagnosis of any of the following:

    • Schizophrenia
    • Schizoaffective Disorder
    • Bipolar Disorder
    • Major Depression Disorder with Psychotic Features

    *As well as other criteria

    These adults experience:

    • Symptoms that are persistent and hard to manage
    • Difficulties meeting basic needs, protecting themselves, keeping safe, sustaining adequate housing, or staying employed
    • Often spend a significant amount of time in hospitals or homeless due to their symptoms
    • Legal problems due to mental illness and co-occurring disorders

  • Research in comparing Assertive Community Treatment to case management shows people who receive services are less likely to be hospitalized and more likely to have stable housing. Studies also show that people who receive services find it more satisfying than case management programs.
